Scene Health connects healthcare workers, patients, and families to improve health outcomes and address medication adherence issues

Scene Health is passionate about bridging the gap between healthcare professionals, patients, and their families to drive better health outcomes. The company’s innovative platform combines video technology, clinical coaching, and evidence-based interventions to dramatically improve medication adherence rates.

Scene Health is revolutionizing the way patients take their medications, tackling the most pressing issue in healthcare: medication nonadherence. It’s a staggering statistic that can lead to hospitalizations, readmissions, and even mortality. There are many reasons why people don’t take their medications as prescribed, including confusing instructions, side effects, or simply forgetting.

Scene Health is passionate about bridging the gap between healthcare professionals, patients, and their families to drive better health outcomes. The company’s innovative platform combines video technology, clinical coaching, and evidence-based interventions to dramatically improve medication adherence rates. With Scene Health, patients can achieve up to 98% adherence in just 90 days. The platform is designed to support patients in taking their medications correctly, improving their overall health outcomes.

By leveraging Scene Health’s platform, health plans can boost member engagement and satisfaction rates, close gaps in care, meet value-based quality metrics, and improve STARS, HEDIS, and CAHPS scores. When patients take their medications as prescribed, they experience fewer hospitalizations, readmissions, and ED visits. Adherence can also help patients avoid disease progression and costly treatments, resulting in significant cost savings.

Scene Health has a particular solution for hypertension, a condition that can have devastating consequences if not managed properly. Nonadherence to hypertension medications can increase the risk of heart failure, stroke, renal insufficiency, and death. The company’s platform simplifies medication management to help patients improve their health outcomes and reduce healthcare costs.

Scene Health’s platform uses video technology to connect patients with healthcare professionals via video conferencing. Patients can discuss their medication regimen, ask questions, and receive guidance on how to take their medications correctly. This helps to build trust and rapport between patients and healthcare professionals, which is essential for improving medication adherence.

The company’s clinical coaching team provides personalized coaching and support throughout the patient’s journey. Patients receive ongoing interventions as needed to address any barriers or challenges that may be standing in the way of adherence. The care team provides ongoing support and monitoring to ensure patients sustain their progress and self-manage their medications.

Scene Health’s evidence-based interventions are designed to support patients in taking their medications correctly. These interventions may include small financial incentives for adhering to treatment plans, motivational interviewing techniques to encourage patients to take control of their medication regimen, and educational video content to help patients understand the importance of medication adherence.

What sets Scene Health apart is its commitment to ongoing support and monitoring. The company knows that medication adherence is not a one-time event, but rather an ongoing process that requires ongoing support and encouragement. That’s why Scene Health provides ongoing coaching and support to patients throughout their journey, helping them to stay on track and achieve their goals.

Scene Health is proud of the results it has seen with its platform. Patients who use the platform are able to achieve up to 98% adherence rates in just 90 days, which is significantly higher than industry averages. And by reducing hospitalizations, readmissions, and ED visits, Scene Health is able to help patients save money on healthcare costs while also improving their overall health outcomes.

At Scene Health, the company is passionate about helping patients take control of their medication management and achieve better health outcomes. It believes that by working together with healthcare professionals and patients, it can create a more effective and efficient healthcare system that prioritizes patient-centered care.
